Eligibility & Referrals

Criteria

INS services are offered to people who have one of the following primary neurological conditions:

  • Stroke, Multiple Sclerosis (MS), Parkinson’s Disease (PD), Brain Tumour, Ataxia, Motor Neurone Disease (MND), Multiple Systems Atrophy (MSA), Primary Supranuclear Palsy (PSP), Head Injury
  • Our geographical range is currently limited to the boroughs of Hounslow and Richmond

Priority will be given to any rapidly deteriorating conditions, such as PSP, MND, MSA.

Referrals for any other primary neurological condition will be considered by our monthly clinical committee. Acceptance will be based on current capacity and the skill base of our existing team to meet the client’s needs.

Please note: treatment interventions may be limited if a patient exceeds the weight limit for our existing equipment – currently 20st/ 112 kg.

Referrals

We have an open referral policy, accepting self-referrals and referrals from other sources, such as consultant neurologists, GPs, health and social care professionals and other community services. Download an INS Referral Form.

Basic personal information completed & consent obtained
Referral forwarded to Assessment team
 
Initial home visit and assessment completed
Goals and aims identified with client and/or carer
 
Case discussed at weekly team meeting
Planned intervention agreed and Keyworker allocated
 
Correspondence sent to GP & Referrer informing of planned INS input
 
Specialist assessments completed, as needed
 
INS services provided
 
Referral made to internal & external services, as required
 
Annual review completed
